Automatic Withdrawal Authorization
Click on the "Automatic Withdrawal" link below and process your payment by using automatic withdrawal from your bank's checking or savings account. For this you will need to fill out the form, print and sign it, and then mail or fax it to us for processing. Please allow two weeks for processing.
