Required forms for your camper to attend our camps:

EQUINE WAIVER & RELEASE OF LIABILITY FORM (PDF) 

Required for campers that are enrolled in the Equestrian/Wrangler program or are signed up for Horseback riding lesson.

HORSEBACK SIGN-UP FORM (PDF)

If you still need to sign up your camper for Riding Lessons (only for those campers enrolled in programs other than Equestrian/Wrangler).

SCUBA

DISCOVER SCUBA DIVING PARTICIPANT STATEMENT, MEDICAL QUESTIONNAIRE AND LIABILITY RELEASE (PDF)

YOUTH DIVING RESPONSIBILITY AND RISK ACKNOWLEDGMENT (FOR 10-11 YEARS OLDS ONLY) (PDF)

Required for campers that are enrolled in or wish to enroll in the Discover SCUBA program.

RSTC MEDICAL STATEMENT PARTICIPANT RECORD (PDF)

This form is to be signed by your physician ONLY if you've answered 'YES' to any question on the Medical Questionnaire.

• Advanced SCUBA Training for boys (forms are print version)

SCUBA FOR ADVANCED TRAINING (PDF)

CONTINUING EDUCATION ADMINISTRATIVE DOCUMENT (PDF)

Required for campers that are interested in Advanced SCUBA training.

This form is to be signed by your physician ONLY if you've answered 'YES' to any question on the Medical Questionnaire.

• Open Water SCUBA Diving Program for boys (4-week session)

For those campers enrolled in the Open Water SCUBA Diving program, the required forms are included in the Student Record Folder that will be sent to you with the Open Water Manual and Logbook.
